Overtraining Syndrome & Burnout

What is Overtraining Syndrome?

In the pursuit of athletic excellence, it’s easy to push your limits. However, if you find yourself training vigorously but feeling worse instead of better, you might be experiencing Overtraining Syndrome (OTS) or burnout. At Sozo, we believe that understanding the balance between training and recovery is vital for sustainable performance and overall health. Overtraining syndrome (OTS) is a condition that occurs when an athlete or active person trains beyond their body’s ability to recover, leading to a long-term decline in performance and overall well-being. It isn’t just normal tiredness — it’s a chronic state caused by excessive training without enough rest, nutrition, or recovery time.

Symptoms of Overtraining

Overtraining can cause a persistent mix of physical, emotional, and performance-related symptoms. People often experience chronic fatigue that doesn’t improve with rest, ongoing muscle soreness, frequent injuries, and a weakened immune system that leads to more illnesses. Performance typically declines despite increased effort, with longer recovery times and reduced strength or endurance. Psychological symptoms are also common, including irritability, low mood, anxiety, difficulty concentrating, and a loss of motivation or enjoyment in training. Sleep disturbances, appetite changes, and hormonal imbalances may also appear, making overtraining a condition that affects the whole body as well as mental well-being.

Investigations

A diagnosis of overtraining syndrome (OTS) is primarily clinical and can often be made based on a detailed history and physical examination. Additional investigations may be used to rule out alternative causes of underperformance or physiological stress. These may include:

  • Blood tests – to assess markers of systemic inflammation, hormonal imbalance, iron deficiency, or infection that may mimic or contribute to symptoms.

  • Cardiac investigations – such as ECG or echocardiography, particularly if symptoms include fatigue, reduced exercise tolerance, or palpitations, to rule out underlying cardiovascular pathology.

  • Performance testing – including VO₂ max or lactate threshold assessment to identify reductions in athletic performance that support a diagnosis of OTS.

Treatment Priniciples

  • Appropriately loading of the tendon to allow healing whilst managing your exercise volume
  • Physiotherapy – Combination of eccentric and concentric exercises (Isometric if pain is severe in the first instance)
  • Exercises are built up weekly until strength is increased and you are able to return to your activities without symptoms
  • Specialised return to activity programs are designed by our team
  • The length of the treatment program will vary but tendinopathies take a while to heal; often in excess of 12 weeks

Many athletes overlook potential underlying factors contributing to their symptoms. Our comprehensive evaluations include:

– Hormonal Imbalances: Assessing hormone levels to rule out any issues that may impact your performance and recovery.

– Anaemia and Thyroid Issues: Evaluating common conditions that can lead to fatigue and decreased athletic performance.

– Relative Energy Deficiency in Sport (RED-S): Identifying energy availability concerns that may be affecting your well-being.

By addressing these conditions, we help you move toward recovery and optimal performance.

Overtraining Syndrome & Burnout at Sozo

A specialised assessment may be recommended in cases where symptoms of overtraining persist despite initial management. This is considered a final step to clarify the diagnosis and guide further care, though it cannot guarantee full resolution, as symptoms may fluctuate even with comprehensive treatment. The aim is to identify the underlying drivers of fatigue and reduced performance—whether physical, hormonal, or psychological—and to support recovery through targeted interventions. This approach ensures that contributing factors such as energy imbalance, hormonal issues, or mental strain are thoroughly evaluated, enabling a clearer pathway toward sustainable performance and long-term wellbeing.
